Hospice and Palliative Care Support Team
Your loved one's care support team is there to best meet their needs, wishes and goals to maintain the highest quality of care that's right for them. Together you'll collaborate on a plan that will incorporate your objectives for care and create an action plan.
The specially- trained support care team will include:
- You | Your loved ones
- Hospice or palliative doctor
- Nurse practitioner | Physician assistant
- Nurse | Social Worker
- Pharmacist | Chaplin
- Physical therapist | Dietician
The support care team will hold an initial team meeting to discuss your loved one's goals, needs and wishes to help maintain the highest quality of care. The plan will then include coordination of care among your support team to help ensure you comfortable and that all your needs are met. The plan will include:
- Treatments to help to alleviate and control pain
- Coordination of care among your support care team
- Help in creating a living will
- Emotional and spiritual support and guidance
